2. Purpose

The data provided through the contact form, by telephone, WhatsApp or email are processed in order to deal with your enquiry, manage your appointment request and maintain the resulting healthcare or commercial relationship. Any health data you provide are processed exclusively for the provision of dental care and the management of your clinical record, in accordance with Spanish Law 41/2002 on patient autonomy.

3. Legal basis

The legal basis is the consent of the data subject (article 6.1.a GDPR), the performance of the healthcare relationship (article 6.1.b) and, for health data, the provision of healthcare (article 9.2.h GDPR) and the legal obligations of the healthcare centre.

4. Retention

Contact data will be kept for as long as the relationship is maintained and, afterwards, for the statutory limitation periods. Clinical documentation will be kept for the legal minimum of five years from the discharge date of each care process, or the longer period established by regional regulations.

5. Recipients

Data will not be disclosed to third parties except where required by law or where necessary to provide the service (dental laboratories, insurers with which you have arranged treatment, financial institutions for treatment financing, IT service providers with access to data, acting as data processors). No international data transfers are made.

6. Rights

You may exercise your rights of access, rectification, erasure, objection, restriction of processing and portability by writing to the postal address indicated or to info [arroba] clinicagoleta.es, enclosing a copy of your identity document. You may also withdraw your consent at any time and lodge a complaint with the Spanish Data Protection Agency (www.aepd.es).

7. Security

4M Clínica Orientada, S.L. has adopted the technical and organisational measures necessary to guarantee the security of personal data and prevent their alteration, loss, unauthorised processing or access.

8. Contact form

Fields marked with an asterisk are mandatory. By submitting the form, the user declares that the data are true and accepts this privacy policy. Children under 14 require the consent of their parents or guardians.
